What is PACE?

PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes. There are 180 PACE programs, serving more than 80,700 participants in 33 states.

The number of people aged 55+ is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise.

The PACE home care RN promotes and maintains participants’ health in the community through education, counseling, and coordination of preventive and rehabilitative services with the interdisciplinary team. The RN assesses health needs, oversees and implements nursing care plans, and provides direct nursing care. The role includes ongoing re-evaluation of individual and family needs. The RN coordinates scheduling, communication, and documentation to ensure efficient home care services.

Experience:

  • One to two years of recent hospital nursing or other acute setting; home health and/or LTC experience preferred.

Licenses and Certifications

  • Current, valid Registered Nurse (RN) license.
  • ·Valid driver’s license, reliable transportation, and proof of required automobile insurance.
  • Must possess and maintain current CPR certification

Knowledge, Skills, and Abilities

  • Ability to set priorities, manage competing demands, and consistently follow through.
  • Strong interpersonal, verbal, and written communication skills.
  • Ability to communicate clearly, effectively, and respectfully in a wide range of situations, including highly emotional or stressful interactions.
  • Proficiency with electronic health record (EHR) systems and Microsoft Office applications.
